Recurrent Atherosclerotic Obstruction of the Internal Carotid Artery Five Years After Endarterectomy • A patient is described in whom an obstructive atherosclerotic plaque recurred in the cervical portion of the internal carotid artery five years after endarterectomy. The lesion appeared to be a new formation of atheromatous material contiguous to the site of the previous arteriotomy and resulted in neurological symptoms almost identical to those experienced by the patient five years before. This case suggests that the treatment of cerebral ischemia by surgical removal of stenotic lesions in the extracranial cerebral arteries may produce only temporary relief of symptoms and emphasizes the need, following endarterectomy, for medical therapy of the underlying atherosclerotic disease. Urgent stenting for patients with acute stroke due to atherosclerotic occlusive lesions of the cervical internal carotid artery.

Acute symptomatic occlusion of the cervical internal carotid artery (ICA) can be treated by intravenous administration of tissue plasminogen activator, percutaneous transluminal angioplasty, and carotid endarterectomy. Carotid artery stenting (CAS) is now indicated for cervical ICA stenosis, but the safety and the efficacy of urgent CAS have not been established. We retrospectively reviewed 10 ...

CASE REPORT Late spontaneous recanalization of internal carotid artery: case report

Late spontaneous recanalization of internal carotid artery is an unusual event that has received little attention. The authors report a case of a 73-year-old male patient, hypertensive, with previous history of cerebral vascular accident 3 years ago, with sensorimotor sequela in the right upper limb. Duplex scanning and arteriography showed total occlusion of the internal carotid artery. The pa...

Recurrent stenosis at site of carotid endarterectomy.

Five patients studied by the authors and 28 mentioned in the literature indicate that recurrent stenosis occurs in no less than 0.6% of patients after carotid endarterectomy. The pathology of the recurrent stenosis was stated in only 10 cases indicating atherosclerotic disease in various stages of development in 7 and a fibrous intimal hyperplasia in 3. Correlation between risk factors for the ...

Long-term results of endarterectomy of the internal carotid artery for cerebral ischemia and infarction.

The immediate and long-term results of carotid endarterectomy for treatment of acute cerebral ischemia or infarction were studied in 95 patients who were observed for an average period of 40 months after the surgical procedure. The operation resulted in death for 12 patients and worsening of the neurological deficit in six others. During the follow-up period, there were 22 additional deaths, mo...

Internal carotid artery stenosis due to atherosclerotic plaque damage after whiplash injury.

Blunt traumatic occlusion or stenosis of the internal carotid artery is a rare complication of whiplash injury and may not be recognized until the onset of neurological symptoms. The clinical course can vary considerably, with regard to both the symptoms and the interval between injury and manifestation. A dissecting aneurysm and intimal tear are usually observed after blunt internal carotid ar...
